Full Time Case Manager
Montezuma County Public Health Department is seeking a Case Manager in the Single Entry Point’s Options for Long Term Care Program. This is a two year position with potential for future employment with a new contractor. Bachelor’s degree in the human behavioral sciences field required. This position requires good communication skills including interviewing and assessment skills, ability to develop service plans, knowledge of long term care community resources, excellent computer & organizational skills along with a desire to help others.

The hiring range for this position is 41,600.00 – 44,100.00 annually.
